If your port lock has been misplaced, damaged, stolen, or you want to upgrade from a serial to a USB port lock, follow the steps below in order to obtain a replacement:
1. Submit to the Customer Service Department (email@example.com) the following:
A. A copy of a police report (Only if port lock was stolen).
B. A copy of the insurance claim (Only if port lock was stolen).
C. A letter on company letterhead stating that the port lock was misplaced, damaged, or stolen. It must also state that need a replacement port lock and if recovered it will be return to FARO. The serial number of the port lock must be listed on this letter. (Only if port lock/computer was stolen).
Note: By sending us as much documentation as possible, it will enable us to process your order in the timeliest manner.
2. Mail or fax a complete Purchase Order to the Customer Service Department for the appropriate charge:
A. $520.00 US Dollars per port lock for the replacement of the port lock(s). (This includes shipping fees)
B. $160 US Dollars per replacement, if the original port lock(s) are sent back to FARO.
3. If the replacement is for a damaged, defective or Serial port lock, you are required to mail your original port lock to:

*Please note that we will be unable to send a replacement port lock until we receive tracking information on the returning port lock.
