You will need to provide the serial number of the Arm, return shipping instructions and possibly payment information.
Once they receive all of this information they will generate a RMA # (Return Material Authorization Number) that you will need to reference on the equipment that is returning. It is recommended that this is scheduled 30 days prior to certification expiration. Then they will need you to review and complete the attached document(s).
NOTE: NO collect or COD shipments will be accepted unless authorized by customer service department in advance!
